We encourage people to send in a petition form
with as many signatures as they can get but if all you can get is
your own, we very much appreciate your support! If you want to get
more than your own signature, please take note of the state laws.
Here's the instructions:
Anyone who has passed their 18th birthday can
circulate the petition. Circulators do not need to be registered
voters, nor do they even need to reside in the state.
TO PRINT OUT THE FORM:

TO PREPARE THE FORM FOR USE:
* Read the circulator affidavit at the bottom and fill in (but
don't sign it yet) the circulator name/address/etc info. Your
signature as circulator should only be added after all other
folks have signed.
* Fill in the county name in the blank at the top.
TO OBTAIN SIGNATURES:
* Important! The only signatures that will actually be counted
as valid are those of voters who are registered in the County
specified at the top of the form.
* Use a clipboard or a thin notebook under the petition to
ensure legible writing. We suggest blue or black ink.

6. ENCOURAGE SIGNERS TO ENTER ALL INFORMATION REQUESTED. Make sure they know to
print everything until the signature. If they hesitate at date of
birth, remind them that this is an official state document and
having complete information ensures voter verification.
7. Ensure signers print the current date, their first
name, middle initial, last name, address and birth date
information. When they sign the petition, they should
sign using their middle initial if they have one.
8. WE SUGGEST YOU START WITH YOUR FAMILY AND FRIENDS.
Then move on to asking your neighbors. If you really want to help
you should contact us directly about joining the community outreach
teams to get petitions signed and spread the word!

Affidavit at the bottom.
